Requirements for Certification or Endorsement

1. The jurisdiction of this office extends to the states of Kansas, Missouri, Colorado, South Dakota, North Dakota, and Nebraska.
2. One (1) completed application form.
3. One (1) photocopy of Taiwan/U.S. passport(s). (Note: Only the page with picture and name is needed).
4. Certification fee is $15.00 per English document being certified. Certification fee is $15.00 per Chinese translation being certified. (NOTE: If Chinese translation is needed, you must provide the Chinese translation and the translator's signature must be notarized by a local notary. This office does NOT translate for you. You must also certify one English document for EACH Chinese translation. Certification fee is thus $15.00 + $15.00 = $30.00 per set of English and Chinese documents.) We only accept money orders paid to the order of TECO.
5. In order to ensure security of documents, Regular mail return envelopes will not be accepted. Fedex, UPS, DHL, U.S. Postal Service have service from our building. If applying is done for by USPS, pre-paid returned postage (priority mail：US$6, Domestic Express Mail: US$18.30 or international express mail: US$38) and self-addressed envelop must be provided upon application.

3. Original diploma (not required if applicant is in Taiwan. Please sign your name on the back of the photocopy and put the words "此影本與正本相符")
4. Photocopies of diploma being certified, plus one (1) extra copy. (For example, if two (2) copies are being requested for certification, send two (2) copies.)

2. Original drivers license (in English), OR COLOR PHOTOCOPY of FRONT AND BACK with the words "此影本與正本相符" and need to be notarized by local notary public.
3. Chinese translation of FRONT AND BACK of drivers license. (For example, if two (2) are being requested for certification, send two (2) copies.).

3. Original paper must be notarized by a local notary and the Secretary of State of the state where the paper is issued.
4. One extracopies of original papers showing notarization form local notary and Secretary of State.

Remarks:
This Office is not responsible for delayed, lost, or stolen items sent by mail at the applicant's request. If the application is done for by mail, pre-paid returned postage (certified, registered or express mail only) and self-addressed envelope must be provided upon application.
Warning: Drug trafficking is punishable by death according to the Criminal Laws of the Republic of China.
