Legalization - Health Certificate (for Resident and Working Holiday Visa)

【General Information】

Consular District:
The consular district of this office includes Greater Ottawa Region (including surrounding cities). We process applications for passports, visas and document authentication within the above-stated areas. Application from other parts of Canada should contact our offices in Vancouver (www.roc-taiwan.org/CA/YVR), Toronto (www.roc-taiwan.org/CA/YYZ), or Montreal (www.roc-taiwan.org/CA/YUL).

9:00AM - 12:00PM, 1:30PM - 4:00PM, from Monday to Friday
The office is closed on statutory holidays.

Payment:
Payment is only accepted by Cash (Please prepare the full amount of fees, no change will be given for mailing and in person application.), Money Order, Bank Draft or Certified cheque (payable to “Taipei Economic and Cultural Office in Canada” or " Taipei Eco. & Cul. Office in Canada"). Personal cheque is not accepted.

NOTE: When you purchase a Bank draft or Bank Cheque, please make sure the cheque contains the Magnetic Ink Character Recognition (MICR) code.

【Required Documents】

	Required Documents
1	Application form for legalization	The completed application form for document authentication filled and sign by the applicant.
2	The valid original passport and one photocopy of the applicant	The original passport will be returned to the applicant after the process. If original passport cannot be provided, a verified true copy of the passport must be obtained from a passport office or Service Canada (for Canadian passports), or the government authorities (e.g., the embassy, commissioner, or consulates) for passports of countries other than Canada and Taiwan.
3	Original Health Certificate	A business card or contact information (including email address) must be provided so that we can contact the physician to verify the certificate. This form is also for application for Youth Mobility Program (Working Holiday).
4	*Return Envelope	Within Canada - A prepaid return envelope Xpresspost from Canada Post - Greater Ottawa Region, Quebec: Please provide Regional return envelope; Newfoundland and Labrador: Please provide National return envelope. *If the Health Certificate is for application for visa, the authenticated Health Certificate will be included in your visa application package; thus you do not have to provide an extra return envelope for this application.
5	Fees	Please refer to Application Fees. Legalization/visa application fees are non-refundable once accepted, regardless of approval, rejection, or withdrawal of the application.
