Note for ROC (Taiwan) visa applicants who have resided in or visited Israel, Afghanistan, Pakistan, the Democratic Republic of the Congo, Guinea, Algeria, Djibouti, Cameroon, or Chad for four weeks or longer in the past year

In accordance with the recommendations of the World Health Organization, all ROC (Taiwan) visa applicants who have stayed for more than four weeks during the past year in Israel, or in other countries or regions at high risk for poliomyelitis (namely Afghanistan, Algeria, Cameroon, Chad, the Democratic Republic of the Congo, Djibouti, Guinea, and Pakistan), are required to provide proof of vaccination against polio.
The vaccination must be administered between four weeks and one year prior to the date of visa application submission and must consist of either the oral polio vaccine (OPV) or the inactivated polio vaccine (IPV). Proof must be provided in the form of an International Certificate of Vaccination or Prophylaxis (yellow booklet) or an official medical certificate or letter issued by a licensed physician or a recognized health provider.
Applicants who are unable to receive the polio vaccine due to medical contraindications must instead submit an official medical certificate or letter issued by a licensed physician or a recognized health provider, confirming that the applicant is medically unfit to receive the polio vaccination.
