DOCUMENTS REQUIRED(Filled out in Chinese if you hold a R.O.C passport)

  1. Application form for authentication.  (Filled out in Chinese if you hold a R.O.C passport) / ODT.
    • Applicants should fill in the deceased's information. If certain information cannot be provided (such as email or phone number), please write 'N/A' or 'None' instead of leaving it blank.
    • In the red-bordered section at the bottom of the application form, please fill in the representative family member's information, ensuring that a contact phone number or email address is provided.
  2. Original and copies of the death certificate issued by the New Zealand Department of Internal Affairs. Authenticate the original; the copy will be kept on file.
  3. Original (to be canceled) and copy of the deceased's Republic of China (Taiwan) passport. The original will be returned after corner-cutting; the copy will be kept on file.
  4. The representative must be a family member of the deceased. Please provide an original and copy of a valid Republic of China passport or other identification documents. The original will be returned after verification; the copy will be kept on file.
    • If your passport has expired, you need to renew it or present the expired passport along with a second document that includes your Chinese name and a photo, such as an ID card, ARC (Alien Resident Certificate), Taiwanese driver's license, household registration transcript, etc. Alternatively, you can provide a valid New Zealand passport or other proof of identity.
  5. Chinese translation version of the death certificate.
    • If a translation of the death certificate is required, the translator must sign in person at our office. The English original must be authenticated before the translation can be verified.
    • If the translator and the representative mentioned in item 4 above are different individuals, the translator must complete a separate application form.
    • If mailing the documents or entrusting someone else to handle them, the Chinese translation must first be certified by a Notary Public within our jurisdiction in New Zealand, and then authenticated by our office. (Please do not use a Justice of the Peace.)
    • If you are entrusting someone else to handle the documents, please submit a letter of authorization.
    • Please note that our office’s authentication only certifies the authenticity of the signature and does not verify the content of the document.
    • Our office needs to review these documents. Providing the email addresses of the Notary Public may help expedite the processing.

FEES

  • NZ$26.00 per document.
  • NZ$13.00 per copy (applicants must provide copies).
  • Payment is accepted only via Eftpos, cash, or Internet banking (details will be provided after receiving the required documents).
  • For mail-in applications, we will email transfer information and document codes after receiving the application. Please be sure to include the document code when making the transfer.
  • International transfers may take several working days to be received. The document authentication fee must be paid in full, with the actual amount received after deduction of bank charges from both countries.

DURATION

  • Once the document has been accepted or confirmed as having been signed by the notary public, it will be issued approximately 5 working days later (excluding holidays and mailing time).
  • Please bring the receipt when collecting the documents. If someone else is collecting the documents on your behalf, they must present both the receipt and their identification.
  • If you need the documents to be couriered back to Taiwan, you must arrange an online appointment with DHL or FedEx yourself and provide the waybill to the processing staff. The documents will be picked up from our office (TECO in Auckland) and sent to Taiwan.
    • For the sender, please fill in this office's address and your name.
    • For the recipient, please fill in the address you wish to send to.
  • For mail returns within New Zealand, please prepare a postage-paid envelope and use a tracked mail service.
  • If the documents are not collected within three months of verification, they will be destroyed in accordance with the regulations of the Bureau of Consular Affairs of the Ministry of Foreign Affairs. No further notification will be given, and the fees paid are non-refundable.
  • Once the documents have been verified and bound, if they are opened or altered by the applicant, the reviewing unit may refuse to process them.

CHINESE VERSION

  • If the relevant authority in Taiwan deems it necessary to provide a Chinese translation, the party involved can prepare the Chinese translation domestically and, along with the foreign original document verified by our office, apply for notarization of the translation with a notary public in Taiwan.
  • When verifying Chinese translation documents, the translator must personally sign the translation at our office. The translation should be completed by the translator themselves and should closely follow the format of the original document.
  • If visiting in person is inconvenient, you may sign the documents in front of a notary public and send them by mail.
