REQUIREMENTS FOR RESIDENT VISA APPLICATION FOR CONTRACT WORKER

The minimum age required for Factory Worker, Operator, Fisherman, etc., is twenty (20) years old, and twenty three (23) years old for Caretaker and Domestic Helper.
(Documents from No.3-15 should be submitted with both original and one photocopy-A4 size)
- An online Visa Application Form (please access https://visawebapp.boca.gov.tw to fill out the Form, then print it out and sign )
- Two (2) passport size photos (45 mm in height and 35 mm in width with white background) taken within 6 months
- Current passport valid for at least nine (9) months and old passports with previous resident visas from the Republic of China (Taiwan), if any.
- Letter of Recruitment and Letter of Visa Permit (both in Chinese) from the Ministry of Labor, R.O.C. (Taiwan).
- Medical Examination Clearance from any of the following accredited hospitals:
- Manila Doctor’s Hospital
- Physicians’ Diagnostic Services Center (Manila and Cebu)
- S.M. Lazo Medical Clinic
- St. Martin Polyclinic, Inc.
- St. Peter Paul Medical Clinic, Inc.
- Philippine Medical Test System
- Sacred Heart Diagnostic Medical Center, Inc.
- MedWay Healthcare Inc.
- Clemen’s Medical Clinic
- Medisense Laboratory Center Inc. (Davao Branch)
- Foreign Worker’s Affidavit for Wage / Salary and Expenses Incurred before Entering the Republic of China (Taiwan) for Employment authenticated by Department of Migrant Workers (DMW)
- Employment Contract authenticated by Manila Economic and Cultural Office (MECO) in Taiwan
- Certificate from National Bureau of Investigation (NBI clearance)
- Special Power of Attorney for PRA authenticated by MECO in Taiwan
- Training Certificate or CAV (Certification Authentication and Verification) for Caretaker issued by TESDA (a must requirement for the first time and for those who worked for less than 6 months; at least 90 hours of related courses taken.) Please note that Certificate in Household Services (NC II) is not acceptable.
- Birth Certificate issued by Philippine Statistics Authority (PSA)(If late registration, please also submit local government birth Certificate)
- COMELEC Voter's Certification or Voter's ID
- Unified Multi-purpose ID or National ID(no other substitute documents will be accepted)
- Affidavit for foreigner to know of the relevant work rules and regulations of the Employment Service Act
- Any other new or old documents and IDs (Travel Records, etc.) deemed necessary by TECO
Visa Fee: P 3,500.00 (NOT REFUNDABLE) payable upon filing of application
Processing Time: 6 Working days
Expedited Service Fee: P 1,750.00

Filing Time: Monday to Friday except holidays; 08:45 a.m. to 11:45 a.m.
Releasing Time: Monday to Friday except holidays; 03:00 p.m. to 04:45 p.m.
NOTE:
- Effective on July 3, 2009, late birth registration done after 21 years old would not be accepted for the first time applicants.
- INCOMPLETE APPLICATIONS WILL NOT BE ACCEPTED.
- This office will NOT expedite the visa application process or issuance time due to migrant workers' flight schedules.
- Migrant Worker Visa applicants whose applications remain unattended in our office for one month from the filing date will be considered as having their applications denied.
- Considering the circumstance wherein a Filipino woman changes her surname to her husband's after marriage or following the nullification of her marriage, this office may request the submission of the following documents: a marriage certificate or a certificate of no marriage(CENOMAR), Philippine entry and exit certificates reflecting both her single name and married name.
