6. Marriage Contract issued by NSO and authenticated by Department of Foreign Affairs and TECO
7. NBI Clearance should be authenticated by Department of Foreign Affairs and TECO
8. Medical Examination Clearance (Form B)(valid only for three months from date of issue) from any of the following accredited Hospitals and Clinics and authenticated by Department of Health and Department of Foreign Affairs.
