寮國文件驗證申請應備資料

一、文件驗證申請表(下載網址 https://www.taiwanembassy.org/vn/post/19839.html)
二、申請驗證之文件: 寮文原文及中或英文譯文須妥善裝釘，並加蓋公證處騎縫圓章，寮文原文及譯文須先經寮國法院公證、外交部驗證、寮國駐越南大使館複驗。當事人應出示文件之正本以供本處核對，正本文件驗畢退還。
三、申請人有效之護照
四、上述文件應備齊正、影本各乙份
五、依照每一種文件的使用目的，本處將個案要求提供其他佐證文件正、影本各乙份包括 : 戶口名簿、戶籍謄本、社會保險、出生證明書、生產證明書、DNA鑑定證明書、認領同意書等其他證明文件；本處因個案需要另函查其他相關單位。
六、規費 :
(一) 普通件 : 寮國駐越南大使館的簽章每個15美元
(二) 急件 : 寮國駐越南大使館的簽章每個22.5美元
七、作業時間:
(一) 普通件 : 4個工作天(不包括送件日)
(二) 急件 : 2個工作天(不包括送件日)

十、申請人倘委託給其他人代為辦理，受委託人須持經公證之合法委託書正本及上揭文件來處申請辦理文件驗證，並須出示受委託人身分證明文件如護照或身分證正、影各乙份。

2. Documents for application for verification: The original Laotian text and the Chinese or English translation must be properly bound and stamped with the notary public’s seal. The original Laotian text and the translation must first be notarized by the Lao Court, verified by the Ministry of Foreign Affairs, and re-verified by the Lao Embassy in Vietnam. The parties concerned shall produce the original documents for verification by this office, and the original documents shall be returned after verification.

Depending on the purpose of use of each document, this office will require the submission of one original and one photocopy of other supporting documents, including household register, household registration transcript, social insurance, birth certificate, production certificate, DNA identification certificate, consent form for claiming and other supporting documents; this office will also contact other relevant units for inquiries as needed.
6. Fees:
(I) Ordinary: The stamp of the Lao Embassy in Vietnam is US$15 per piece
(ii) Urgent: The stamp of the Lao Embassy in Vietnam is US$22.5 per piece
7. Working time:

If the applicant entrusts another person to handle the application, the authorized person must bring the original notarized legal power of attorney and the above documents to apply for document verification, and must also produce the authorized person's identity documents such as passport or ID card and a photocopy.

10. ຖ້າຜູ້ສະໝັກມອບໝາຍໃຫ້ຜູ້ອື່ນເປັນຜູ້ຈັດການການຮ້ອງຟ້ອງ, ຜູ້ມີອຳນາດຕ້ອງນຳເອົາໃບປະກາດສະນີຍະບັດຕາມກົດໝາຍສະບັບຕົ້ນ ແລະ ເອກະສານທີ່ກ່າວມາຂ້າງເທິງນັ້ນມາຍື່ນຂໍຢັ້ງຢືນເອກະສານ, ພ້ອມທັງຕ້ອງສ້າງເອກະສານຢັ້ງຢືນຕົວຕົນຂອງຜູ້ມີອຳນາດເຊັ່ນ: ໜັງສືຜ່ານແດນ ຫຼື ບັດປະຈຳຕົວ ແລະ ສຳເນົາເອກະສານ.
